If you have questions regarding our payment and co-payment requirements, vision plan or insurance billing, missed or cancelled appointment guidelines, or any other financial or office policy, please do not hesitate to ask a staff member to clairfy the information for you. Payments, Co-Payments, Deductibles
We accept cash, personal checks, VISA, MasterCard, Discover Card, American Express, Care Credit, and all bank cards that display the VISA or MasterCard logo.
All payments, co-payments, and deductibles will be collected at the time services or materials are provided. If special payment arrangements or extended payment considerations need to be made, we offer Care Credit as an alternative financing option.
Care Credit is a medical credit card company that can provide zero-interest, low-interest, and other extended payment options to those that qualify. You can apply online, or ask us for an application in the office.

Vision Plans, Medical Insurances, Pre-Authorizations
We are aware that understanding your vision plan or medical insurance benefits can be difficult. We will try to help answer your questions to the best of our knowledge and understanding; however, no guarantee of accuracy regarding your benefits can be made by anyone other than your vision plan or medical insurance carrier. It is always helpful when you understand the nature of your vision plan or medical insurance benefits prior to your office visit.
We will accept assignment of benefits and submit any necessary claims on your behalf for all pre-authorized services and materials to your vision plan or medical insurance carrier. Please keep in mind that this service is not a guarantee of payment. If your vision plan or medical insurance carrier does not reimburse, or does not cover, claims submitted by our office, then as the vision plan or medical insurance subscriber, payment in full is your responsibility.
In the special case of office visits without prior pre-authorization from a vision plan or medical insurance carrier, payments will be collected in full at the time services or materials are provided. Assignment of benefits will not be accepted; however, for your consideration we will submit any necessary claims on your behalf to your vision plan or medical insurance carrier. In this scenario, reimbursment from your vision plan or medical insurance carrier will be made directly to you.

Missed or Cancelled Appoinments
If you are unable to keep your scheduled appointment with us, we require a 24 hour notice to cancel or reschedule.
As a courtesy to all our patients, we do not double-book appointments. We schedule uninterrupted time between you and the doctors so that you can have the most comprehensive appointment possible. When you miss or cancel your appointment without notice, or on the same day of your appointment, we are almost always unable to give that time to other patients.
Please help us maintain a convenient schedule for everyone by keeping your scheduled appointment, or by providing us with at least 24 hours notice if you are unable to make it. In the unfortunate circumstance of chronic missed or cancelled appointments, a $50.00 fee may be assessed to the account holder.
