Like many other modern medical offices, we are in the process of implementing Electronic Medical Records (EMR). EMR's help streamline the record-keeping process by storing vital patient information, exam data, and other necessary records digitally. By employing an EMR system, we are able to track the progress of your healthcare more efficiently and provide better personalized healthcare, while at the same time reducing the overall waste of natural resources.
However, in order to ensure the most accurate record-keeping as possible, it is necessary for us at times to update our permanent records. This means filling out a paper form. We are working hard to go completely "paperless," but please be understanding if you are asked to fill out a Patient Intake form or update your Health History record at your next visit.
If you are new to the office, please take a moment to print out and complete the Patient Intake and Health History forms below for each member of the family. In order to help expidite your initial visit, please bring these completed forms with you on the day of your scheduled appointment. Please read the forms carefully and be as detailed as possible.
